COVID Monoclonal Antibodies: Do They Work in Dialysis?
Monoclonal antibody bamlanivimab appeared to work for end-stage renal disease (ESRD) patients in early data from dialysis providers, but there are plenty of questions that remain.
The first 40 dialysis patients who got the antibody treatment at U.S. Renal Care clinics had a 7-day hospitalization rate of 15% compared with 57% among a randomly sampled comparator group of 49 coronavirus-infected dialysis patients matched for age, gender, diabetes status, and race.
The 30-day hospitalization rate was 39% versus 65% for those patients treated from when the dialysis network first started administering the drug January 5 through the end of February.
That represents almost the totality of their experience with the drug, said Mary Dittrich, MD, chief medical officer of U.S. Renal Care, who reported their data at an American Society of Nephrology webinar earlier in March.
With falling case counts nationwide, only another five patients have been dosed since, she told MedPage Today. “A tradeoff that I’m very willing to make is that our infections have gone down, so our administrations have gone down.”
Fresenius Medical Care North America and DaVita Kidney Care have also been tracking their numbers and results, with their larger patient populations.
Across DaVita’s more than 220 infusions, no adverse events were reported with the infusion or in the 1-hour observation period after it, George Aronoff, MD, vice president in DaVita’s Clinical Affairs Office, told MedPage Today.
“Only about 9% of our patients who have received the infusion have required hospitalization within 21 days following infusion,” he noted.
Fresenius reported similar results: “Since offering this antibody treatment in our dialysis centers in early January, our preliminary data suggests that less than 10% of patients who received the therapy were hospitalized within the first 28 days after treatment,” said its chief medical officer, Jeffrey Hymes, MD.
Altogether, it appears that bamlanivimab — developed and sold by Eli Lilly — safely reduced progression of disease in the ESRD population, Dittrich said. “It’s small numbers and it’s early, but given the paucity of any data in our population…that’s the data we’re excited about.”
Few, if any, ESRD patients were included in those pivotal clinical trials, so some data is better than none, she noted.
“It has been gut-wrenching to be helpless in this pandemic,” Dittrich said. “To have any option both in terms of vaccines and these monoclonal antibodies is inspiring.”
However, there are some big cautions with this observational data. So much so that her group decided not to publish, she said.
Monoclonal antibodies have to be given as outpatient treatment after a positive for COVID-19, but there are logistical issues, Dittrich noted.
A large portion of dialysis patients are admitted soon after COVID-19 diagnosis, typically within 5 days. In U.S. Renal’s data, the average time to bamlanivimab administration after diagnosis was 4 days.
“There is inherent bias,” she warned. “It’s very possible that we selected for a certain group of patients to get bamlanivimab — those with milder disease, those who were going to stay outpatient for 4 days.”
Hymes noted that the early Fresenius data will also require further analysis to correct for possible baseline differences in the characteristics of patients who did and didn’t get the antibody.
Another issue is that the data only included bamlanivimab monotherapy, whereas distribution of that drug has stopped due to reduced efficacy against SARS-CoV-2 variants such as those identified from California and New York. Instead, the combination of bamlanivimab and etesevimab (also from Eli Lilly), and a two-antibody cocktail developed by Regeneron Pharmaceuticals, are the ones being distributed.
“I do worry about how the variants rising in prevalence is going to affect the efficacy of this therapy, and that’s one thing we’ll be watching closely,” Dittrich said. “I don’t know that we have the ability to genomically sequence all our patients, but we want baseline efficacy data out there, knowing that the variants may change that data.”
Fresenius continues to advocate use of monoclonal antibodies for appropriate patients, Hymes noted, with recently updated clinical guidelines to emphasize the availability of the combination antibody and the discontinuation of bamlanivimab monotherapy.
Monoclonal antibodies are still relevant even as vaccination will be rolling out to ESRD patients through dialysis clinics nationwide in the next few weeks, Dittrich said.
“With the variants and the country opening back up, we worry that numbers are going to go back up,” she said. “Especially if these variants cause more severe disease, which there is some suggestion they do, this will be even more relevant.”

COVID forces India’s former Gulf workers to forge new futures
It is not yet dawn but Yeroor village is long awake, the hum of productivity floating over “Gulf Street”, a lush green boulevard named for the thousands of workers who leave the southern Indian state of Kerala every year for jobs in the Middle East.
But now the workers are back, from machine operator Sudheesh Kumar, who has been forced back into manual labour in Yeroor to make ends meet, to former banker Binoj Kuttappan, who has taken up dog breeding in the state capital, Thiruvananthapuram.
In the single biggest reverse migration in more than 50 years, workers from the Gulf have streamed back to the coastal state of Kerala in the past year, propelled by a pandemic that deflated dreams of overseas riches and changing family fortunes.
While once they came home wealthy and revered, bearing gold, sunglasses, clothes and funds to buy homes, now they have returned sheepish and penniless.
“Prior to COVID, they were celebrated as heroes. Now they have nothing,” said Irudaya Rajan, a professor at the Centre for Development Studies in Kerala, who has studied migration patterns in India’s southernmost state.
“This is the first time they have returned empty-handed and will end up borrowing and selling assets,” said Rajan.
Among the Indian states, Kerala sends the most workers to the Gulf, accounting for about 2.5 million of six million Indian nationals there.
The state received about 19 percent of $78.6bn transferred to India from overseas workers in 2018, the highest state tally in the country that is the world’s top recipient of remittances.
But more than 1.1 million people have returned in the last 10 months, 70 percent having lost their jobs as domestic workers, builders, waiters, chefs and more, official data shows.
This has wrecked workers and their families’ lives and destroyed businesses dependent on the India-Gulf migration.
Kumar, 50, spent 22 years in the Middle East, with his final job in Saudi Arabia operating machines at Jeddah airport’s wastewater treatment where he earned triple the average Kerala wage.
In March 2020, he flew home – briefly, he thought – but flights were grounded in a bid to contain the new coronavirus.
Now, the father of two splits his time between farm labour and working in a stone quarry in the village of 13,000 people.
“I had planned my life when I left 22 years ago. I had any ordinary man’s dreams – a house, good education for my children,” a deflated Kumar told the Thomson Reuters Foundation outside his house, sweat beading his brow.
Kumar has been forced to sell his car and farmland to pay off a loan for his four-bedroom home in Gulf Street.
Now he is earning 400 Indian rupees ($5.50) a day compared with a fixed monthly 20,000 rupees ($272) in Jeddah with overtime on top.
“I have no shame in doing hard labour, but how did I land here? Where did I go wrong?” Kumar said.
During the Gulf War 30 years ago and the 2008 financial crisis, many workers were forced back to Kerala, but this time the numbers are far higher number and the job market tighter.
A nationwide initiative linking returnees with jobs has notched up more than 30,000 registrants, about 80 percent of them from the Gulf States of Bahrain, Kuwait, Qatar, Saudi Arabia, United Arab Emirates and Oman, according to a government release.
Shamna Khan, 30, whose right leg is badly swollen by lymphoedema, never needed to work because her husband Shafir sent enough money home from his job in a glitzy Qatar mall.
The couple turned their mud and clay house to concrete, laid tiles, built an indoor toilet and got help for Shamna’s leg.
But after Shafir returned jobless last March, Shamna registered for India’s rural job scheme for about 300 rupees ($4) a day that guarantees a minimum 100 days of work in their village such as building roads, digging wells and trenches at farms.
“I am happy to work as I can support my family, but my leg is prone to infections,” said Shamna as she dug a trench at the village’s rubber plantation.
Sharif, who works at the quarry, worries about the looming uncertainty, his unpaid loans – and Shamna’s health.
“There is no other work here,” Sharif said.
More than 90 percent of Indian migrant workers, most of whom are low and semi-skilled workers, work in the Gulf region and southeast Asia, according to the United Nations.
Connecting them with jobs are recruitment agencies and travel firms which match workers with employers and book them on flights – a hectic business that Ajimon Mak, 45, nurtured for 14 years in Thiruvananthapuram.
“Ticketing was my main business, it was a passion and I was always busy. During the lockdown I saw it all go down to zero,” said Mak, who moved into fishmongering and recently opened a fish shop in Thiruvananthapuram.
Former banker Binoj Kuttappan, 40, also forged a new path after returning from Abu Dhabi last year following layoffs at his financial service company and decided to turn his passion for dogs into a breeding business.
“I would have never done this if not for the pandemic,” said Kuttappan, showing off seven dogs he bought for 150,000 rupees ($2,044).
With plans for a pet accessories shop, a garden for dogs and air-conditioned kennels, he has no plans to return to the Gulf – but others are counting the days until they can go back.
Kumar has started calling agencies seeking work in the Gulf.
“My savings for our future are gone and now our future looks bleak,” said Kumar. “I no longer think of making a profit. I only think of surviving the day.”

In the United Kingdom, where hesitancy is low, more than 80% of people recently surveyed about their willingness to get a vaccine said they had already been vaccinated or planned on it. In Germany, those who say they probably or definitely would get vaccinated is up to nearly 70%. In France, around half of people have said they would get a COVID-19 vaccine. The ICL team modeled these countries as “low,” “medium,” and “high” hesitancy scenarios, respectively, and they compared those results with an ideal scenario — 98% of people 15 and older are vaccinated.
The ideal scenario in the model assumes that a few people can’t take the vaccines because of allergies or preexisting health problems, but that everyone wants to get the vaccine.
Model Predicts Winter Surges
The model predicts several future surges in COVID-19 cases — mostly over the winter months — continuing into 2024, as people resume travel and return to offices and schools, restaurants, and entertainment venues.
Even in a scenario like that in the UK, which has highly effective vaccines and hesitancy is low — perhaps between 10% and 20% — the model shows that daily deaths will be nearly 9 times higher at the peak of the first surge because people opt out on vaccines. Over the next 2 years — through the end of 2022 — the model predicts about 30% more COVID-19 deaths in a low-hesitancy country like the UK, about 350% more deaths in a medium-hesitancy country like Germany, and about 770% more deaths in a country with high hesitancy like France.
Where does the United States fall in that mix? The ICL team didn’t model the US specifically, but recent surveys suggest that America is most closely aligned with Germany in the “medium” hesitancy position.
With medium hesitancy, the model showed 1744 deaths from COVID-19 per million people — with about 168 per million of those deaths in vaccinated people and 1576 in unvaccinated people, through the end of 2022.
Tricky to Model
There are some things to keep in mind about this model, though, said Rupali Limaye, PhD, director of behavioral and implementation science at the Johns Hopkins Bloomberg School of Public Health, Baltimore, Maryland.
She says hesitancy is hard to model because it clusters. Whether or not someone is likely to get the vaccine depends on a slew of factors, including what their friends post on Facebook, their race, age, gender, political affiliation, religion, and even their level of education.
“People that tend not to get vaccines tend to hang out with each other,” she said.
For the purposes of the model, the ICL team only factored age into the hesitancy equation. They assumed that in every other way, people were equal.
Limaye said the numbers in the ICL model look reasonable, but in real life, the deaths and hospitalizations as a result of hesitancy won’t be evenly spread. Some communities will be hit much harder by hesitancy than others.
Just knowing the numbers doesn’t help you intervene where it’s most needed.
Limaye, who wasn’t involved in the ICL modeling, has been working with Johns Hopkins colleague Shaun Truelove, PhD, to try to make similar estimates for the US.
Truelove, a data modeler, said the ICL report painted a picture that was perhaps too pessimistic.
First, he said, vaccine hesitancy has declined in the US and other countries.
“More and more people are getting vaccinated. We’ve got more data on safety, more data on effectiveness; and we also, at the same time, have had this re-emergence of the pandemic with the variants,” Truelove said. “Because of all that, willingness also goes up.”
He did point out that willingness could drop if people lose confidence in some of the newer vaccines, like AstraZeneca’s.
The model looks forward almost 2 years. Truelove said that’s a long time when you’re talking about such a fluid situation. When he’s making models, he doesn’t like to go any farther out than about 6 months.
Also, he said, models aren’t forecasts, but they can be useful to help scale potential impacts and help guide public policy decisions.
The study authors say they wholeheartedly agree on that point.
“Getting vaccinated is an individual choice; however, this choice has social consequences,” said Daniela Olivera Mesa, a PhD student at Imperial College London, in a news release. “Our work demonstrated that vaccine hesitancy can have a substantial health impact that affects both the vaccinated and unvaccinated populations. Building trust in vaccines is an important public health priority to control COVID-19.”
